Go to USAhockey.com
Click on Register on line (it is on the right hand of the upper part of the page)
Click on Ice Players and Coaches
Click the box that states you are 18 or older
Then hit Continue
Click on the 2009/2010 box
Click on a Child or Family Member Under 18
Begin to complete the form
When putting your phone number in do not use the dashes (-)
Click that you Accept the Waiver
Put your initials (it only takes two)
Click to Continue to Checkout
Click Continue if you are only doing one
Complete the billing information
Click Pay Now
Open and print the certificate – Make one copy for your records
And give a copy to your team manager/Board Member.
